After significant weight loss: what non-invasive treatment can recover, and what it cannot

L'équipe Bloom Clinic

Reaching your target weight does not always give you the silhouette you pictured. Three distinct problems hide behind that, and they are not treated the same way.

The weight is reached, and the silhouette does not look like what was hoped for. What then arrives in consultation is not one problem but three, regularly confused: skin that has not retracted, muscle lost along with the fat, and a face that has hollowed. Each calls for a different answer, and non-invasive treatment does not recover all three.

Why this request has become so common

Large and rapid weight loss is markedly more common than it was a few years ago — whether it comes from a change of diet, from exercise, from surgery or from medical follow-up. With it came a new request, and almost always the same one: "I succeeded, and I don't recognise myself." This is not vanity. Stretched skin and a slackened abdominal wall are visible, and they are lived with.

First problem: the skin has not retracted

Skin has a real but limited capacity to retract, depending on age, on how fast the weight came off, and on how much there was to make up. The faster and larger the loss, the less it follows.

This is the ground of HIFU and radiofrequency: both heat tissue at depth to contract existing collagen and stimulate new. The result builds over two to three months, it is gradual, and it has a ceiling — we come back to that.

Second problem: the muscle went with the fat

This is the one that is least well identified, and often the most decisive. Rapid weight loss costs muscle as well as fat. And it is muscle that holds a silhouette: a stomach can be flat and still look hollow, simply because the abdominal wall no longer supports anything.

No technology that targets fat will correct that — there is no fat left to target. EM-Sculpt, on the other hand, triggers muscle contractions of an intensity impossible to achieve voluntarily: muscle is what it works on, and muscle is exactly what is missing.

Third problem: the face

The face loses fat like everything else, and it needed that fat. Two things happen there at once: the skin slackens, and volume decreases. HIFU and radiofrequency answer the first. For the second, let us be clear: restoring volume is not a matter for non-invasive treatment, and it is not what we practise. A centre promising to "refill" with a device is telling you a story.

The limit you need to hear

Faced with significant excess skin — an abdominal apron, arms that frankly hang — no non-invasive technology will give a satisfying result. It will give a partial one, and you will have paid for a disappointment. Removing skin is a surgical act, and saying so beforehand rather than afterwards is part of the job.

Between the two lies a wide zone where non-invasive treatment does real work: moderate laxity, skin lacking tone, a silhouette to firm rather than to rebuild.

The calendar trap

Wait until the weight is stable. Treating skin while the loss is still ongoing means working on a moving target: what has been tightened will slacken again, and the course will have to be repeated. A few months of stability are worth more than a series of sessions begun too early.

Where to start

  1. A body assessment, to separate what belongs to skin, to muscle and to volume — all three look alike to the naked eye.
  2. Muscle first if the abdominal wall is at fault: it is what restores a line, and its effect shows soonest.
  3. Skin next, as a course, with a result judged at two or three months rather than on the way out of a session.
  4. Drainage alongside, without expecting any contouring effect of its own.
  5. And an honest opinion on whatever exceeds non-invasive treatment — the most useful part of the consultation.

In Rabat and in Casablanca, the assessment at the first consultation at Bloom Clinic is free. After weight loss it serves one purpose above all: telling you which of these three problems you actually have, and what can be done about it.
